Last write wins by revision (the client's edit time, epoch ms): when the server already holds a NEWER revision (written on another device) nothing is changed and that version comes back with applied: false. The target must exist and be visible to the caller at write time (else 404). Throttled per member. Other open devices hear draft.updated on the caller's private channel (key + revision only, never the text).

Not authenticated. code: account_disabled means the member's seat was deactivated in this workspace: the session is over even though the token was not revoked (clients sign out and clean up).
