Revoke one of my MCP keys

Deletes one of the acting member's MCP keys; the agent holding it gets 401 on its next request. Another member's key answers 404. Audited as mcp_key.revoked.

Not authenticated. code: account_disabled means the member's seat was deactivated in this workspace: the session is over even though the token was not revoked (clients sign out and clean up).
