Make an appearance template

Starts from base — a ready-made template (preset:card@1 or card@1; default the platform default) or another template's id — with tokens (a change) on top. Guard rails (422): only known keys and listed values (no key styles one consent button alone — Accept all, Reject all and Preferences share one style); a banner layout only at a position it can take; every colour a visitor reads at 4.5:1 on its background, in each palette the template uses (the error names the nearest colour that passes). Names are unique per kind (case-blind), at most 50 templates per kind. Doubles as the import of an exported template (send its tokens). Audit appearance_template.created.
