Call the contact from the member's PBX extension
POST/api/contacts/:contact/pbx-calls
Click-to-call: the PBX rings the member's own extension first and dials
the contact once they pick up. Requires pbx.use and an extension
assigned to the member. A member's click is a transactional call (no
İYS question, the same rule as calls on ConnectWiz lines);
commercial: true asks the commercial-consent gate for ARAMA first.
422 with reason no_extension, no_phone, a consent code
(iys_no_consent, opted_out, …) or extension_unknown; 429 when
the PBX asks us to slow down; 502 when it cannot be reached.
