Update a knowledge source

Edit a knowledge source's title and training material (admin only). Editing a text / qa source triggers a re-ingest: the old chunks are deleted and the new content is chunked + re-embedded under the SAME source id. For a file source the body is the title (always) plus an OPTIONAL new file: supply a file to re-ingest, omit it to rename only. The source's type is IMMUTABLE — to switch types, delete the source and create a new one. The endpoint accepts both application/json (text / qa) and multipart/form-data (file).

Not authenticated. code: account_disabled means the member's seat was deactivated in this workspace: the session is over even though the token was not revoked (clients sign out and clean up).
