Turn an accepted offer into a sale

The lines come from the quote's snapshots, not from the catalogue: what the customer agreed to is what gets billed, even if somebody has since renamed or repriced the item.

One sale per quote, enforced by the database. Converting twice would double the sale in every report that reads this table, and the mistake looks exactly like a good month.
