Make something unavailable for a window

Always recorded as manual — the source is set by the server and never taken from the request, because a block claiming to come from a connected system would be overwritten by that system's next sweep without warning.

ends_at must be strictly after starts_at. A zero-length block would render as a reason with no effect, and somebody would trust it.
