The call-id keyed form of the conversation reject. A decline that lost the race to an answer stays a no-op: the response carries the call's current state rather than cutting a live conversation.

Not authenticated. code: account_disabled means the member's seat was deactivated in this workspace: the session is over even though the token was not revoked (clients sign out and clean up).

Authenticated but not allowed.

404

Resource not found (or not visible to the current tenant).

The call has already ended, or its channel is no longer configured.
