Approve a transfer request (AC-4)
POST/api/transfer-requests/:transferRequestId/approve
A supervisor approves a pending request: the conversation is assigned to
the resolved target (an optional to_user_id override, else the requested
teammate) or released to auto-routing. Gated by the assign power.

Not authenticated. code: account_disabled means the member's seat was deactivated in this workspace: the session is over even though the token was not revoked (clients sign out and clean up).
