Connect selected Meta assets

Create channels for the SELECTED discovered assets. Admin only. The access tokens are read back from the server-side pending store by external_id (never sent from the browser); each created channel is best-effort subscribed to the platform app. Already-connected assets are skipped; the pending entry is cleared afterwards. An expired / unknown pending_id → 422.

Not authenticated. code: account_disabled means the member's seat was deactivated in this workspace: the session is over even though the token was not revoked (clients sign out and clean up).
