Store the CRM status mapping (admin)

/api/integrations/crm/:provider/status-mapping

Save what a human decided. Only the values of CrmAppointmentState / CrmQuoteState are accepted — anything else is a 422 carrying the machine key unknown_state and nothing is written. A token sent blank or null is CLEARED rather than stored as an empty string.

The write never calls the CRM, so a decision is storable while the provider is down. Answers with the whole screen, so a save and a reload are indistinguishable.
