Upload a media-header sample

Upload a sample image / video / document via the Resumable Upload API and return the handle to reference as header_handle when creating or editing a media-header template. Admin-gated (Channel update). asset_id sends a media-library file instead of file (doc 60 §7).

Not authenticated. code: account_disabled means the member's seat was deactivated in this workspace: the session is over even though the token was not revoked (clients sign out and clean up).
